Bilingual (Spanish) Social Worker – Chen Med

• Conduct psychosocial assessments and identify social, financial, and family needs for patients with chronic or life‑threatening conditions.
• Advocate for Medicaid eligibility and assist patients with benefit applications.
• Link patients to internal and external resources, prioritizing high‑risk patients on the HPP and Top 40 lists.
• Maintain accurate, timely documentation per billing and company guidelines.
• Collaborate with the care team, attending meetings such as weekly Super Huddles and Transitional Care Team meetings.

• Licensed Social Worker (LCSW/BHS) or equivalent certification.
• Experience with Medicaid and community resource coordination.
• Strong knowledge of social services documentation and community health agencies.
• Excellent communication, critical thinking, and teamwork skills.
• Ability to work autonomously and navigate complex patient situations.
• Fluency in Spanish and English, with written and spoken proficiency.
