Responsibilities- Conduct psychosocial assessments and identify social, financial, and family needs for patients with chronic or life‑threatening conditions.
- Advocate for Medicaid eligibility and assist patients with benefit applications.
- Link patients to internal and external resources, prioritizing high‑risk patients on the HPP and Top 40 lists.
- Maintain accurate, timely documentation per billing and company guidelines.
- Collaborate with the care team, attending meetings such as weekly Super Huddles and Transitional Care Team meetings.
- Licensed Social Worker (LCSW/BHS) or equivalent certification.
- Experience with Medicaid and community resource coordination.
- Strong knowledge of social services documentation and community health agencies.
- Excellent communication, critical thinking, and teamwork skills.
- Ability to work autonomously and navigate complex patient situations.
- Fluency in Spanish and English, with written and spoken proficiency.
