Care Management Specialist

$14.00/hr - $16.00/hr

Shifts Available

1st, 2nd, and 3rd shifts available.

Responsibilities

Under the direct supervision of the CMS Supervisor, Care Management Specialists are responsible for all matters relating to the safety and security of employees, residents, and visitors. They ensure the continual accountability of assigned residents and a safe and secure environment for treatment and rehabilitation. CMS supervises resident drug and alcohol testing, visitation, recreation, laundry procedures, cleaning, transportation services, and other related duties as required.

CMS staff participate in other facility activities that foster a safe and secure environment conducive to the goals and mission of Community Assessment & Treatment Services.

Credentials / Academic Requirements
  • Proof of a high school diploma or equivalent is required.
  • At least one (1) year of experience working cooperatively with others in a human service setting.
  • Ability to consent to and pass a criminal background check and drug screen.
  • Demonstrates knowledge of Substance Use Disorder (SUD) treatment & recovery, consumer engagement, healthcare, social services, and criminal justice systems.
  • Experience with adult offender/inmate/resident supervision and discipline preferred.
  • Interpersonal communication skills require courtesy and tact when dealing with other staff, criminal justice agencies, local organizations, and the general public. Must be able to deal with residents and their families in a tactful, yet firm, fair, and consistent manner.
  • Ability to relate with people, listen, and then resolve problems effectively; ability to define problems, collect data, establish facts, and draw valid conclusions.
  • Experience with handling emergencies and potentially dangerous situations; ability to remain cool under pressure; ability to deal with aberrant behavior.
  • Ability to communicate effectively in writing; read, understand, and write reports, memos, and policies.
  • Basic understanding of crisis intervention techniques.
  • Ability to work independently, without supervision.
  • Ability to use Microsoft Word, email, and agency's electronic medical record system.
Preferred Qualifications /Skills
  • Training or experience in Substance Use Disorder (SUD) treatment & recovery, consumer engagement, healthcare, social services, and criminal justice systems.
  • Ability to consent to and pass a criminal background check and drug screen.
  • CDCA preferred.
  • Experience with adult and/or juvenile offender/inmate/resident supervision and discipline preferred.
  • Valid Ohio Driver’s License preferred.
  • Must be able to obtain a Medicaid number.
