Licensed Social Worker (OH / PART-TIME / Field-based)

Freedom Care, a leading provider of home care services in Ohio, is seeking a Licensed Social Worker (LSW) to join our Structured Family Caregiving (SFC) program. This is a part‑time, hybrid role requiring field work and commuting to our Cleveland, OH office at least twice a month.

• Interpret and communicate the company’s philosophy and objectives to clients, families and partners.
• Ensure compliance with HIPAA, Ohio Department of Medicaid, Ohio Department of Aging and all agency policies.
• Conduct initial in‑person home assessments to establish compliance and care plans.
• Provide ongoing monitoring, including monthly contact with patients and caregivers, covering enrollment, satisfaction, and program integrity.
• Educate and coach caregivers, maintaining accurate case records and documenting changes promptly.
• Serve as escalation point and advocate, collaborating on plan of care with physicians and addressing any issues.
• Perform additional duties as directed by the supervisor.

• Valid, active Ohio Licensed Social Worker license issued by the CSWMFT Board.
• Bachelor’s degree (BSW) or Master’s degree (MSW) in Social Work from a CSWE‑accredited program.
• Minimum two years of clinical experience, including one year working with elders or adults with disabilities.
• Experience in home care and/or use of CRM (Salesforce a plus).
• Bilingual in Spanish is a plus.
• Valid driver’s license and insured vehicle for state travel.

• Demonstrated success in driving operational excellence and measuring outcomes.
• High adaptability, experience in case management, and ability to thrive in a fast‑paced environment.
• Excellent verbal and written communication skills with the capacity to cascade information across diverse audiences.
• High emotional intelligence, empathy, and customer‑centric mindset representing Freedom Care holistically.
• Experience using a CRM (Salesforce a plus).
