The Professional IV—Behavioral Health Therapist participates in the clinical team by providing care and care coordination to individuals with mental health needs. Provides integrated care by ensuring that whole person's health needs are met, including primary and behavioral healthcare, as well as substance abuse services. Works collaboratively with other team members. Uses care coordination skills with individuals, their families, and both internal and external providers to ensure seamless care.
Links client to health promotion and wellness activities and helps to make linkages to community services. Maintains a therapeutic relationship with clients on his/her team in meeting the biopsychosocial needs of the client.
- Provide care coordination and integrated care to individuals with mental health needs, including primary and behavioral healthcare and substance abuse services.
- Collaborate with multidisciplinary team members to ensure seamless client care.
- Use care coordination skills with clients, families, and internal/external providers; connect clients to health promotion, wellness activities, and community resources.
- Maintain a therapeutic relationship with clients to address biopsychosocial needs.
- Conduct mental health assessments and provide ongoing counseling and psychotherapy using evidence-based practices for individuals, groups, and/or families.
- Perform level-of-care assessments and determine care needs (including screening measures for level of care, drug and alcohol treatment needs, and long-term counseling needs).
- Provide community psychiatric support and care coordination, including health promotion, transitional care, family support, and referrals/advocacy for community resources.
- Develop and review individualized treatment plans; update plans as needed; communicate vital treatment information to the treatment team.
- Collaborate with Nurses, Physicians, and Pharmacists to achieve positive client health outcomes; assist clients in obtaining and maintaining comprehensive healthcare.
- Attend medical appointments with clients and transport or accompany as needed; provide ongoing communication to clients, healthcare teams, and support networks for follow-up and coordinated treatment.
- Complete required administrative tasks including clinical records, progress notes (within 24 hours of service), daily tracking, and weekly billable hours; perform other duties as assigned.
- Master’s degree in social work or counseling.
- Licensure or credentialing: LSW, LPC, LISW, or LPCC.
- A valid driver’s license in the State of Ohio with access to an insured vehicle for client transportation.
- Ability and desire to work closely with other team members.
- Ability to form effective therapeutic relationships to engage clients in mutual assessment of problems and goal setting, and to coordinate care plans from initial contact through end of service.
- Excellent oral and written communication skills.
- Ability to make sound decisions under stressful conditions.
- Demonstrated clinical skills with the ability to organize and prioritize tasks.
- Awareness of and sensitivity to the cultural diversity of the populations served.
- Preferred: prior experience working with SPMI adults.
- Ability to understand and accept a team approach to service delivery, and agency/program philosophy and goals.
