Position Overview
The Community Health Worker (CHW) is responsible for helping patients and their families navigate and access community services, resources, and adopt healthy behaviors. The CHW supports providers and the care team members through an integrated approach to care management and community outreach. Activities will promote, maintain, and improve the health of patients and their families by addressing social determinants of health (SDOH) that impact client health.
The CHW provides social support, informal counseling, and advocates for individual and community health needs.

- Understanding of care plans and carrying out deliverables.
- Understanding of social determinants of health screening tools: PRAPARE, ACE.
- Establish trusting relationships with patients and their families while providing general support and encouragement.
- Provide ongoing follow‑up, basic motivational interviewing.
- Follow up with patients via phone calls, home visits, and/or other settings where patients can be found.
- Help patients set personal health–related goals and attend appointments.
- Provide referrals for services to community agencies as appropriate.
- Help patients connect with transportation resources and provide appointment reminders.
- Effectively communicate NFP’s mission and work closely with medical providers.
- Record patient care management information in the EMR and other software within 2 business days of patient contact.
- Manage assigned caseload of patients.
- Comply with and understand all HIPAA and confidentiality requirements at all times.
- High School Diploma or equivalent required.
- Active Community Health Worker Certification approved by the Ohio Board of Nursing.
- 1 to 2 years' experience in a community‑based healthcare setting preferred.
- Basic computer skills required; electronic medical records (EMR) experience preferred.
- Previous experience with EPIC EHR system preferred and understanding of the community served and community connectedness.
- Good communication skills such as active listening and effective oral communication.
- Ability to speak multiple languages proficiently, English and Spanish strongly preferred. Test may be required pre‑hire.
- Ability and willingness to provide emotional support, encouragement, and motivation to patients.
- Ability to communicate effectively with all internal and external customers.
- Flexibility to work in office and in the community as needed and attend various meetings and events.
- Able to sit, stand, bend, and move for extended periods of time.
- May involve lifting materials up to 30 pounds.
- Ability to work in a busy office environment with many interruptions.

Evaluation:
This position will be evaluated based on its ability to effectively engage with patients and their families, providing the necessary support and resources to improve their health outcomes. Success will be measured by the quality of relationships established with patients, the timeliness and thoroughness of follow‑up care, and the ability to navigate and connect patients with appropriate community resources.
