Access Specialist is often the first point of contact for new and returning patients for our school-based program. This position responds to and screens incoming referrals, handles data entry, coordinates consent and access to care, and completes, updates or reviews diagnostic assessments. The role primarily serves school-aged patients and their families across office, school, and community settings and works as a licensed mental health provider within a collaborative team.
Responsibilities- Provide trauma-informed services to patients and their family system across office, school, community, home, and virtual environments, including intake, consultation, assessment, and triage services.
- Maintain accurate, concise, and concurrent documentation according to program procedures, including diagnostic assessments, diagnoses, risk assessments, progress notes, and intake documents.
- Interview all new clients to conduct thorough diagnostic assessments.
- Provide and coordinate crisis intervention and triage services.
- Develop and maintain positive relationships with referral sources and community providers.
- Bachelor's Degree in Social Work;
Master's degree in Social Work/Counseling or related field (preferred). - LPC/LSW/MFT required.
- Experience performing diagnostic assessments in a mental health setting (preferred).
- Experience in crisis intervention, triage, and knowledge of community resources (preferred).
- Experience working in or with educational settings (preferred).
- Must have an acceptable driving record (4 points or less), reliable transportation, and carry automobile insurance with minimum liability limits of $100,000/$300,000 and property damage of $50,000.
