Under general supervision, provides interventions and services outlined by community health worker core competencies to support patients of assigned clinic and surrounding community by promoting health, reducing disparities, improving access to healthcare, and reducing social drivers of health. Services and interventions include screening patients for social determinants of health to identify social needs and connecting identified at‑risk patients with eligible services and resources to address health‑related social needs.

Builds trusting relationships and partnerships with patients, caregivers, care team, and community organizations. Understands the socio‑economic needs and culture of the communities served to identify barriers to individual health and to advocate for changes to improve community health. Upholds the mission, vision, values, and customer service standards of The Metro Health System.

Required:

• High school diploma or passage of a high school equivalency exam.
• One year of experience working with clients in a community‑based/social service setting.
• Obtains Community Health Worker certification from an accredited program within one year of hire.
• Life experiences reflective of the community/target population served.
• Reliable transportation to travel to designated community/clinic locations.
• Ability to work with diverse client populations, community organizations, and staff at all levels.
• Ability to work independently and collaboratively within a team from general instruction.
• Excellent verbal and written communication skills.
• Outstanding interpersonal, administrative, and organizational skills.
• Highly skilled in using personal computers, including, but not limited to, Microsoft Office applications.
• Ability to interact effectively with a wide range of cultural, ethnic, racial, and socioeconomic backgrounds.

Preferred:

• Active Community Health Worker (CHW) certification from an accredited program.
• Additional coaching courses or certifications relevant to role.
• Bilingual.
• Experience in healthcare system or ambulatory clinic.

May sit, stand, stoop, bend, and ambulate intermittently during the day. May need to sit for extended periods. See in the normal visual range with or without correction. Hear in the normal audio range with or without correction. Finger dexterity to operate office equipment is required. Ability to communicate in person, by phone, email, and other communications. Ability to see computer monitor and departmental documents.
