Description

The Referral Specialist at Neighborhood Health Care Incorporated is responsible for managing and coordinating patient referrals to ensure seamless and efficient access to healthcare services. This role involves collaborating with healthcare providers, patients, and insurance companies to facilitate the referral process and ensure that patients receive timely and appropriate care.

Key Responsibilities
  • Coordinate and process patient referrals to specialists and other healthcare services.
  • Communicate with patients, healthcare providers, and insurance companies to gather necessary information and documentation for referrals.
  • Maintain accurate and up-to-date records of referrals and follow‑up appointments in the electronic health record system.
  • Monitor the status of referrals and ensure that patients attend scheduled appointments.
  • Provide exceptional customer service to patients and healthcare providers by addressing inquiries and resolving issues related to referrals.
  • Collaborate with healthcare teams to identify and address barriers to accessing care.
  • Ensure compliance with all relevant regulations and guidelines related to patient referrals.
  • Participate in quality improvement initiatives to enhance the efficiency and effectiveness of the referral process.
Required Education
  • Preferred:
    Bachelor's degree in healthcare administration, social work, public health, or a related field.
Required Experience
  • Minimum of 2 years of experience in a healthcare setting, preferably in a referral coordination or patient navigation role.
  • Demonstrated experience working with diverse populations and understanding of community health resources.
  • Experience with electronic health records (EHR) systems and patient management software.

Skills and Abilities
  • Strong organizational skills with the ability to manage multiple tasks and priorities effectively.
  • Excellent communication skills, both verbal and written, with the ability to interact professionally with patients, healthcare providers, and community partners.
  • Proficient in Microsoft Office Suite (Word, Excel, Outlook) and comfortable learning new technology tools.
  • Ability to work independently and collaboratively within a team environment.
  • Strong problem‑solving skills and attention to detail to ensure accurate and timely referrals.
  • Understanding of healthcare regulations and privacy standards, including HIPAA compliance.
