Hybrid School Based Counselor (LSW, LPC, MFT) – Chrysalis Health

Join us to apply for the Hybrid School Based Counselor (LSW, LPC, MFT) role. This is a hybrid school-based position primarily serving students in a school district with opportunities for home, office, and fully remote via Telehealth.

Base Pay Range

$55,000.00 – $55,000.00 per year

About the Role

As a clinician at Chrysalis Health, you will provide individual and family therapy to a diverse population, including children, adolescents, adults, and families. Therapy sessions will be conducted primarily in schools and may also serve clients at home, in office, or via telehealth. Providers must be licensed in the State of Ohio.

Impactful Client Care Responsibilities
  • Assess and determine the comprehensive mental health needs of clients with an array of patient ages and diagnoses.
  • Conduct and document client screenings, psychosocial assessments, progress notes, and referral information.
  • Deliver services to clients in the community, particularly in the assigned School District.
  • Utilize evidence-based practices to enhance clinical outcomes of clients.
  • Identify resource requirements necessary to fulfill identified needs.
  • Collaborate with caregivers, physicians, school counselors and other healthcare professionals to ensure optimal client care.
  • Advocate for patient access to community resources.
  • Communicate treatment plans and progress to patients, families, and other healthcare professionals.
  • Facilitate and offer individual and family therapy on psycho-social issues such as coping skills, stress reduction, and caregiver support.
  • Demonstrate professionalism to reassure patients of their care by a skilled and compassionate professional.
Candidate Requirements
  • Must be licensed in the State of Ohio as a Social Worker (LSW, LISW, LISW‑S) or Professional Counselor (LPC, LPCC, LPCC‑S) or Marriage and Family Therapist (MFT, IMFT) with a minimum 1‑year experience providing services to persons with mental illness and/or substance use conditions; children, adolescents, adults, and families preferred.
  • Must like and have experience working with children, adolescents, and teenagers.
  • Understanding of developmental trauma, family systems, and somatic experience, or willing to learn.
  • Compassionate, engaging, and have a strong desire to help others.
  • Provide weekly individual and family therapy utilizing evidence-based practices.
  • Complete and submit required documentation within certain time frames.
  • Attend monthly clinical supervision.
  • Ability to flex hours to meet client scheduling needs.
  • Organizational and time allocation skill abilities, and to flex hours to meet client scheduling needs.
  • Ability to utilize self‑care techniques to best manage to achieve optimal work‑life balance.
  • Ability to document therapy services in electronic medical records (EMR) platform.
