The Occupational Therapist in Home Health is responsible for assessing and evaluating patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments, adaptive equipment, and other occupational therapy needs as defined by medical conditions.

Provides services within the scope of practice defined by state laws, in accordance with the plan of care, using evidence-based techniques, and coordinating with other healthcare team members.

Evaluates the patient’s functional status, all relevant body systems for CMS documentation, and occupational therapy needs. Consults with physicians to develop the therapy plan of care.

Observes, records, and reports patient responses to treatment and any changes in condition to the nurse supervisor and physician.

Conducts patient assessments to evaluate functional ability using diagnostic and prognostic tests.

License Requirements:

• Current Occupational Therapy license in the state of practice
• Current CPR certification
• Valid driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation
