• Serving as a liaison between health and social services and the community
• Facilitating access to services and improving the quality and cultural competence of service delivery
• Increasing health knowledge and self-sufficiency through outreach, education, counseling, support and advocacy
• Helping patients navigate healthcare and social service systems
• Managing care and care transitions for vulnerable populations
• Ensuring cultural competence among healthcare providers

• Minimum qualifications are KCSE D or KCE Div. 2 or the equivalent
• Course open to beginners, Community development workers, Welfare officers and Fresh students
