Responsibilities include:

• Managing and improving health for patients with complex and chronic conditions.
• Leading a multidisciplinary team and discussing care plans.
• Developing transitional care plans across the continuum of care, including hospital discharges.
• Facilitating appropriate discharges and communicating with care facilities.
• Treating acute exacerbations of chronic conditions and managing urgent appointments.
• Monitoring patient progress to prevent ER visits and readmissions.
• Practicing evidence-based medicine within the PCMH model.
• Educating patients on disease progression and advanced directives.

Qualifications:

• Licensed Physician in the state of practice.
• Board certified in Internal Medicine or Family Practice.
• BLS certification, DEA registration.
• Mission-driven to serve underserved populations.
• Team-oriented with experience in EMR and problem-solving skills.

Preferred:

• Experience in population health management.
• Experience with complex psychosocial and medical patient populations.
