Delta Health Center, Inc. (DHC) is the first community health center of its kind. It is also the first Federally Qualified Community Health Center (FQHC) in the United States. DHC is a non‑profit organization located in historic Bolivar County, Mississippi, where it opened its doors in 1965. Licensed Practical Nurse (LPN)

• Triage patients
• Expedite patient referrals to specialty offices, answer patient and specialty office calls and complete referral requests
• Maintain ongoing tracking and documentation on referrals
• Work independently and unsupervised while maintaining a superior work ethic
• Ensure that referrals are addressed timely
• Remind patients of their appointments

• Certificate of completion for Licensed Practical Nurse
• Must be IV certified
• Experience in healthcare: 1 year (Preferred)
• Experience in nursing: 1 year (Preferred)
• Preferred licensing: LVN or LPN
• General computer skills required (Microsoft Outlook and Excel)
