• Active, unencumbered LPN/LVN license in the state where the role is based
• Compact licenses must be transferred to the state of residence within 90 days
• Current CPR certification (hands‑on component) – Aveanna will assist if needed
• TB skin test updated within the last 12 months
• Minimum six months of hands‑on nursing experience (preferred but not required)
• Reliable transportation
