We have cared for patients with serious and terminal illnesses throughout North Central Texas, providing compassionate care for those living with an illness while supporting their families. Our mission is to support patients and caregivers with high quality hospice care.

Provides personal hygiene care to hospice patients following the patient's Plan of Care.

• Provides personal hygiene care to patients according to the individual Plan of Care, such as bathing and shampooing.
• Delivers routine medical supplies as needed to patients.
• Observes and reports any changes in the patient's condition to the Nurse/Care Manager.
• Demonstrates respect and understanding of the philosophy, mission, and service standards of CHOT and provides excellent customer service.
• Successfully completes 12 hours of in‑service training annually.
• Assists patients with transfers as needed.
• Provides compassionate companionship to patients while delivering services.
• Depending on the census, assignments may include patients in the field or inpatient units.

• HS Degree or GED.
• Proof of State Certification – Certified Nurse Aide.
• 1 year of direct patient care experience required.
• Demonstrates skills and written competency prior to working independently with the patient.
