• The Case Manager Registered Nurse interacts with patients, physicians, and other members of the healthcare team to assure appropriateness of patient placement and education.
• They will provide outpatient and telephonic education, monitoring, and appropriate interventions to identify high risk inpatients.
• They will encourage appropriate utilization of services.
• They will promote timely discharge.
• They will provide education and in‑services to members of the healthcare team when necessary.
• They will prepare and furnish reports to hospital committees and leadership.
• They will participate in problem solving activities.
• They will perform other duties as assigned.

• Location: Morton Plant Rehabilitation Hospital - Clearwater, Fl
• Status: full time
• Monday - Friday 8:00 AM - 4:30 PM
• Occasional weekend requirement
• On Call: No

• Required RN (Registered Nurse) License – Florida
• Required BLS (Basic Life Support)
• Preferred Case Management Certification

• Required Associate Degree in Nursing
• Preferred Bachelor’s Degree in Nursing

• Required 1 year Utilization ~ OR ~
• Required 1 year Acute Care ~ OR ~
• Required 1 year Home Care
• Preferred 3 years Nursing.
• Preferred 3 years Education.
