The Care Facilitator is a patient’s first contact with the Chen Med family and serves as a patient advocate delivering exceptional customer service. The incumbent checks patients in to and out of the clinic and provides basic secretarial and administrative support. A successful Care Facilitator is committed to superior service and builds trusting relationships with patients.
Essential job duties / responsibilities- Interacts with patients, records information, greets, checks in, checks out, or schedules appointments. Conducts lobby checks to maintain organization and cleanliness.
- Acts as the gatekeeper for PCP and Specialist schedules, including ensuring top patients are scheduled regularly.
- Uses computer systems to add new patient information, print relevant forms, schedule appointments, respond to emails, and generate New Patient packets. Ensures patient and emergency contact information is accurately documented.
- Conducts outreach calls and mails birthday and greeting cards for patients on special occasions.
- Answers telephones, takes detailed messages, and directs messages to the appropriate individual.
- Clearly communicates information about Chen Med clinical personnel to patients and others.
- Verifies patient insurance via phone or web, collects copays, and assists with scheduling follow-up and in-house Specialist referrals as needed.
- Escorts patients to exam rooms and ensures comfort and well-being prior to leaving.
- Acts as HEDIS champion for the PCP by providing visibility of HEDIS needs for scheduled patients and reporting on hospitalizations and related metrics.
- Present patients with customer service surveys at checkout and escalate for immediate service recovery when needed.
- Participates in daily and weekly huddles to provide details on patients.
- Regulates and monitors transportation aspects, including driver lists, patient transport, medication and grocery deliveries, and coordinating Uber/Lyft rides for patients.
- Maintains the confidentiality of patients’ personal information and medical records.
- Reviews patients’ personal and appointment information for completeness and accuracy.
- Transmits correspondence by mail, email, or fax.
- Performs other duties as assigned and modified at manager’s discretion.
- Exceptional customer service skills and a passion for serving others
- Working knowledge of medical insurance and/or electronic medical record (EMR) systems
- Knowledge of Chen Med products, services, standards, policies and procedures
- Proficient in operating phones, personal computers, software, and other basic IT systems
- Professional and courteous communication with employees, patients and others
- Detail-oriented with strong accuracy in reports and data
- Excellent verbal and written communication skills
- Strong listening skills
- Positive and professional attitude
- Proficient in Microsoft Office Suite (Excel, Word, Outlook)
- Ability and willingness to travel locally and regionally up to 10% of the time
- Spoken and written fluency in English
$14.30 - $20.42 hourly
The posted pay range represents the base hourly rate or base annual full-time salary for this position. Final compensation depends on factors including experience, education, geographic location, and other relevant factors. This position may be eligible for bonuses or commissions.
