RN Care Manager

Advocate for older and/or disabled adults and their loved ones to help make decisions related to healthcare, aging and long-term care planning.

• Recognize and advocate for the client's values and wishes, rather than your own
• Conduct, develop, monitor, and manage care plans and assessments to identify issues and problems, eligibility for assistance, and need for services
• Onboard and sign up new clients, educating them on how our Care Managed Home Care (CMHC) services will help them age in place
• Review financial, legal, and/or medical issues and offer referrals and recommendations to specialists and other professionals
• Focusing on avoiding future problems and conserving assets
• Assist with determining the level of care needed and moving a client to a more appropriate facility or arranging in‑home care
• Act as liaison to responsible parties and professionals
• Facilitate the Care Managed Home Care model by managing intakes, completing on‑site safety assessments, and completing care plans for new clients
• Provide coaching for caregivers with new clients to ensure success
• Oversee the orientation and training of new and existing caregivers
• Complete documentation requirements accurately and timely
• Participate in marketing efforts with the goal of educating the public, cultivating clients, and securing new clients

Compensation: $30.00 - $35.00 per hour
