Summary
Position provides support to persons served in PARC's Community Integration Services program. Responsible for monitoring the safety and well-being of program participants and assisting in the delivery of health and wellness, recreation and leisure, community integration, social development, volunteerism, employment readiness, and independence skill development.

Duties

- Provides support and training to small groups of persons served to ensure their well-being, safety, and overall necessities are met while following program guidelines.
- Supports and assists persons in service with personal care, mobility, positive behavior interventions, and the self-administration of medications.
- Helps persons served to identify and achieve the goals they set for themselves and facilitate active participation in a wide variety of meaningful activities to assist program participants to increase their independence, inclusion, and experiences.
- Implements individual goals as indicated on participant Individual Support Plan and complete documentation of progress and performance daily including: participant evaluations, assessments, support strategies, behavior support plans, progress notes, monthly summary reports, daily notes, incident reports and other data as needed.
- Implements program support functions including (but not limited to) skills development classes, free time activities, group and individual community activities, community volunteer activities, recreational and virtual activities. Monitor meals, breaks, work, and recreation areas to ensure safety of program participants at all times.

Responsibilities
- Provide crisis intervention support to participants as needed. Report every incident to the Program Manager immediately and complete incident reports within 24 hours.
- Ensure ongoing compliance with Human Rights, Medicaid, CARF and funding source requirements and regulations.
- Monitor and assist program participants to safely board and exit their mode of transportation.
- Transport program participants to and from events and activities in the community, monitor and assist participants to safely board and exit their mode of transportation.
- Provide feedback for participant goals, positive behavior approaches, personal enrichment, and social integration.
- Assist in the development and implementation of curriculum that focuses on building the skills necessary to support independence and integration.
- Participate in department meetings, participant meetings, and staff meetings as assigned to support program development and implementation.
- Complete in an accurate and timely manner and in accordance to standard operating procedures, all assigned documentation, attendance rosters, incident reports, etc.
- Perform other responsibilities as assigned.
Qualifications:
Education, Experience, and Certification(s)
- High school diploma or General Educational Development (GED) required.
- One year experience working with adults with disabilities is preferred.
- Valid driver's license and/or access to reliable transportation to perform work-related travel required. Eligible drivers must have a good motor vehicle record (MVR).
- Ability to pass and maintain Department of Human Services background clearance required.
Knowledge, Skills, and Abilities
- Physical ability to assist individuals with toileting, mobility, transferring, etc.
- Creativity and enthusiasm to provide broad supports to persons in service.
- Knowledge of behavior support principles and practices.
- Knowledge of the community and its resources.
- Ability to use good judgment and decision-making skills.
- Familiarity with Microsoft Office and the ability to become familiar with corporate specific programs and software.
