Tailored Care Manager

Job Overview

We are looking for a compassionate and committed Tailored Care Manager to be part of our supportive, multidisciplinary Tailored Care Management (TCM) team in Johnston County. This role focuses on delivering whole-person, integrated care to individuals impacted by substance use and mental health challenges.

• Evaluate client needs and develop personalized care plans.
• Coordinate services that address physical and mental health, social determinants of health, and other life challenges.
• Travel throughout Johnston and surrounding counties to close gaps in care.

• Competitive salary ranging from $52,000 to $54,000 annually.
• Full-time Monday–Friday schedule 9 AM–5 PM with rotating on‑call responsibilities.

• Bachelor’s degree in a health, psychology, sociology, social work, nursing, or related human services area. Master’s degree preferred.
• Eligible for Qualified Mental Health Professional (QMHP) credentialing.
• Minimum 2 years of experience with mental health and/or substance use populations.
• Proficiency in Microsoft Office and Electronic Health Records (EHR).
• Valid driver’s license, current auto insurance, and clean driving record.
• Strong communication skills and ability to work independently.
