Tennessee State Veterans’ Home (TSVH) – Clarksville is a fast-paced, high-quality long-term care facility proudly serving our nation’s veterans. Our facility ranks in the top 3% nationally according to CMS and is recognized as a leader in nursing home care. We are seeking dedicated LPNs for full-time and PRN opportunities. A $15,000 retention bonus is offered for eligible positions.

Position OverviewThe Licensed Practical Nurse provides direct resident care in accordance with physician orders, established policies, and standards of practice. The LPN works closely with RN Supervisors, Certified Nursing Assistants, and other members of the interdisciplinary team to ensure high-quality, resident-centered care. This role includes leadership responsibilities and opportunities for professional growth and advancement.

Key ResponsibilitiesProvide safe, effective nursing care to residents throughout the shift in accordance with care plans and provider orders.
Administer medications and treatments accurately and timely.
Monitor residents’ conditions, report changes to the RN Supervisor and provider, and document appropriately.
Collaborate with RN Supervisors, CNAs, and other staff to coordinate care and ensure residents’ physical, emotional, and psychosocial needs are met.
Participate in care planning, shift handoffs, and interdisciplinary team discussions as needed.
Support and model a positive, respectful, and resident-focused culture.

QualificationsCurrent, active LPN license in the state of Tennessee
Previous long-term care experience preferred (1–3 years); new graduates will be considered
High school diploma or equivalent
