* Job Summary:

** Skilled Nursing

** Rehabilitation
* * setting. This individual will play a key role in ensuring residents receive timely access to their healthcare providers through high-quality telehealth support. The ideal candidate is patient-centered, organized, and comfortable working in a clinical environment.
*
* Key Responsibilities:

*** Facilitate and manage resident telehealth visits and scheduling, ensuring timely access to providers.
* Prepare residents for virtual appointments, including setting up equipment and ensuring privacy.
* Communicate professionally with residents, staff, and providers to ensure a smooth telehealth experience.
* Assist with documentation, appointment coordination, and follow-up tasks as needed.
* Support clinical teams by escalating urgent issues and helping residents communicate their health concerns effectively.
* Maintain telehealth equipment and troubleshoot basic technical issues.
* Demonstrate excellent time management to coordinate multiple daily appointments.
* Uphold professionalism, compassion, and customer service in all interactions.

* Qualifications:

*** Preferred:
Background as a CNA, MA or LPN.
* Preferred:
Experience in a Skilled Nursing Facility (SNF) or rehabilitation setting.
* Strong customer service skills and ability to communicate clearly with residents and staff.
* Knowledge of clinical environments and workflows.
* Excellent time management, organizational skills, and reliability.
* Comfortable with technology and able to learn new systems quickly.
