Full-Time/Day Shift
The Clinical Pharmacist Specialist will collaborate with physicians and staff to provide coordinated, comprehensive, high quality care to patients.

Required Qualifications:
• Bachelor's Degree - In PHARMACY from an accredited School of Pharmacy or,
• Ph.D.
- In PHARMACY from an accredited School of Pharmacy.
• Coursework/Training - Postgraduate Year One (PGY-1) AND Year Two (PGY-2) Pharmacy Residency or equivalent experience or equivalent educ/experience.
• Oregon Pharmacist License upon hire.
• Within 2 years of hire:
Board Certified by the Board of Pharmacy Specialties specific to area of practice.
• Pharmacy preceptor license in applicable state as applicable upon hire.
• 2 years - TWO years of Senior Ambulatory Clinical Pharmacist experience within relevant practice setting - OR - FIVE years of pharmacy practice experience with at least
• 2 years of experience in relevant setting in lieu of PGY-1 and PGY-2 residency requirement.
