Position

Full Time Physical Therapist Case Manager

Anticipated Territory: Sacramento County:
Citrus Heights and Surrounding cities

In partnership with other clinicians on the case, you will provide skilled services to evaluate and address the needs of individuals in their homes so they can live safely and productively in their home environment.

Your goals as a Physical Therapist are to restore function, prevent or minimize disability following disease or injury, and help the client reach maximum performance within their capabilities. You will evaluate and treat patients using medically prescribed physical therapy programs. You may supervise Physical Therapy Assistants (PTAs) per agency and regulatory standards. Other responsibilities are described below.

What you’ll do as a Home Health Physical Therapist Case Manager
  • Perform an evaluation in the home and develop a care plan, and work with PTAs to follow the care plan.
  • Help patients with mobility, movement, and regain independence.
  • Create detailed treatment and care plans for each patient.
  • Communicate and coordinate with patients’ physicians to ensure a quality level of care.
  • Evaluate and report patients’ progress to the MD.

To be a success as a Physical Therapist
  • Current unrestricted CA Physical Therapist License
  • Current BLS/CPR Certification
  • Valid CA Driver's License
  • Knowledge of local community resources
  • Understanding of how illness/injury and culture affect health care
  • Knowledge of state and federal homecare regulatory guidelines (preferred)
  • Home health care experience (highly preferred)
  • Experience with high-acuity patients (a plus)
  • Experience working with an interdisciplinary team (highly desired)
  • Case Management fundamentals (highly desired)
  • Self-directed, empathetic, critical thinker with outstanding interpersonal skills
  • Leadership and organizational skills

Salary: $120,000 - $135,000 a year. Wage details: PT Level 1 up to $120,000 and PTI/PTII progression with bonuses; starting pay commensurate with experience. 21st Century offers comprehensive benefits, bonus plans, and 401k with up to 4% match. Advancement to PTII with higher salary and bonus is possible based on supervision of PTAs and case load.
