Founded in 1967, The Health Care Connection was Ohio’s first Federally Qualified Health Center (FQHC). Our mission is to provide quality, culturally sensitive and accessible primary healthcare services. THCC is proudly recognized as a Level 3 Patient Centered Medical Home (PCMH), the highest level of recognition attainable for quality care.

We boast two primary care locations and 6 school-based health centers providing quality value-based care for over 20,000 patients. We provide services in Primary Care, Infectious Disease, Substance Use, Integrated Behavioral Health, Dental Services, Women’s Health, and Pharmacy.

We are currently seeking aLicensed Practical Nurse (LPN) - Pediatric for our Lincoln Heights location. This full-time role is essential in supporting clinical operations and delivering compassionate, patient-centered care.

• Assist providers with direct patient careand clinical procedures.
• Conduct patient interviews and accurately document medical information in the Electronic Health Record (EHR).
• Perform routine screenings and testsas ordered by the provider.
• Handle telephone inquiries, provide clinical advice within scope, and conduct follow-up as needed.
• Ensure exam roomsare properly stocked, sanitized, and equipped.
• Coordinate referrals and pre-authorizations, and communicate next steps to patients.
• Maintain patient confidentiality and comply with all HIPAA regulations.

• Graduate of an accredited
  
  Licensed Practical Nursing (LPN) program
• Current
  
  LPN licensein the State of Ohio
• Active CPR certification
• Preferred:
  Minimum ofone (1) year of LPN experience in a community health, hospital, or private practice setting
• Strong communication and patient service skills
• Ability to work effectively in ateam-based, fast-paced environment
• Familiarity with
  
  EHR systemsis a plus
