We’re looking for a compassionate and self-motivated Registered Nurse (RN) Case Manager to provide in-home visits and case management for clients throughout Cuyahoga County

• Visit clients in their homes to provide assessments and wellness checks
• Create and follow individualized care plans that meet each client’s unique needs
• Partner with families, physicians, and other care providers to ensure seamless coordination

• Active RN license in Ohio
• At least 1 year of clinical experience (home health or case management preferred)
• Driver’s license and reliable transportation
• Strong assessment and communication skills
