Palliative Nurse Clinician – Hospice of Cincinnati
Tri Health is seeking an experienced RN to serve providers in an outpatient palliative care clinic. A minimum of 4‑5 years of acute or ambulatory experience is required; experience in palliative care or hospice is preferred. The position is Monday‑Friday, no weekends or holidays, and requires some local travel between sites.
Job OverviewThe Palliative Care Nurse Clinician provides clinical support for patients with serious illness within the multidisciplinary palliative care team. Responsibilities include collaboration in team consultations and rounding, documentation of assessments and daily rounding, education of patients, families, and staff regarding goals of care and symptom management, and mentorship of nurse students and medical residents.
Responsibilities- Support palliative medicine referrals by providing timely, thorough clinical and psychosocial information.
- Clarify or initiate conversations about illness and treatment options with patients and families.
- Ensure optimal communication and positive relationships among the palliative team, physicians, residents, nurses, and other disciplines.
- Complete documentation in the electronic medical record in a timely manner.
- Respond promptly to voicemails, emails, and other communications.
- Educate patients and families per the Ohio Nurse Practice Act and Tri Health policy about disease progression, medication management, and end‑of‑life issues.
- Support care‑planning meetings and facilitate problem‑solving, goal setting, and decision‑making for patients and families.
- Initiate appropriate referrals to chaplain, hospice liaison nurse, or other disciplines when indicated.
- Collaborate with care‑management staff on discharge preparation and, if needed, make post‑discharge contacts with patients or families.
- Provide case‑by‑case education to other professional staff and mentor safe use of opioids and other analgesics.
- Promote understanding of palliative medicine through teaching moments and formal presentations.
- Document accurately and in a timely manner assessments, interventions, plans, and outcomes, and review outcomes with the team to improve practice.
- Critique palliative and hospice research, incorporate new information into practice, and identify research needs.
- Assess knowledge and skill needs, set professional growth goals, and pursue educational opportunities.
- Provide constructive feedback to peers regarding their practice.
- Associate’s Degree or Diploma in Nursing.
- BSN must be obtained within 5 years of hire (for new hires).
- Hospice & Palliative Nurse Certification must be obtained within one year of hire.
- Registered Nurse license.
- Highly motivated, self‑directed professional.
- Effective interpersonal skills.
- Ability to develop relationships with patients, physicians, and other team members.
- Effective verbal and written communication skills.
- Preferred membership in an associated national professional organization.
