We are seeking a friendly and compassionate PCP Nurse Practitioner to join our team. As a PCP Nurse Practitioner, you will play a crucial role in providing comprehensive care to patients in long‑term care facilities. • Provide comprehensive healthcare services and coordinate patient care
• Work closely with a team of healthcare professionals
• Assess, diagnose, and treat residents' physical and mental well‑being
• Conduct thorough patient assessments, including medical history reviews and physical examinations
• Order and interpret diagnostic tests
• Develop accurate diagnoses and treatment plans
• Prescribe medications and administer treatments
• Provide counseling to residents and their families
• Collaborate with the interdisciplinary team for seamless coordination of care
• Manage chronic conditions and monitor progress
• Make appropriate adjustments to care plans
• Commitment to person‑centered care
• Create a safe and nurturing environment for elderly residents in the nursing home.
• Document billable patient care services into an electronic health record system using accurate ICD‑10 & CPT codes
• Possibly rotating to more than one facility near Millersburg, PA

• Nurse Practitioner License
• DEA License
• 6 months experience in long‑term care
