Clinical Hospice Director -Registered Nurse ( RN

management/supervisory experience, even if it is not in hospice and want to lead. It is a good opportunity to join a reputable, mission driven hospice agency for an experienced Clinical Hospice Director will:

• Provide the supervision, management, coordination and provision of quality patient care
• Collaborate with the physician, referral sources, Clinical Director and professional staff regarding patient eligibility for the hospice program
• Coordinates hospice care services from admission through bereavement.
• Coordinates with physicians, case managers, the patient, families and interdisciplinary team to implement that patient plan of care.
• Coordinates regular interdisciplinary team meeting regarding the patient care process to ensure quality of care and coordination of services.
• Participates in agency quality assurance activities designed to improve quality and continuity of patient care
• Oversees staffing, training and evaluating staff.

• 3+ years as a nurse and I year plus in patient care of hospice, home health or oncology patients.
• 1+ year of supervisory preferably in hospice or home health care.
• Demonstrated ability to provide a supportive approach to staff and patients, along with their families.
