About

the role

The PRN Registered Nurse Case Manager plans, organizes, and directs hospice care and is experienced in nursing, with an emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

Job Responsibilities
  • Completes an initial, comprehensive assessment of the patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es).
  • Provides professional nursing care by utilizing all elements of the nursing process.
  • Assesses and evaluates patient’s status by writing and initiating a plan of care, regularly re-evaluating patient and family/caregiver needs, and participating in revising the plan of care as necessary.
  • Initiates the plan of care and makes necessary revisions as patient status and needs change.
  • Uses health assessment data to determine nursing diagnosis and develops a care plan that establishes goals based on nursing diagnosis, incorporating palliative nursing actions and involving the patient and family in planning.
  • Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the plan of care.
  • Counsels the patient and family in meeting nursing and related needs and provides health care instructions as appropriate per assessment and plan.
  • Assists the patient with activities of daily living and supports self-sufficiency and comfort care.
  • Acts as Case Manager when assigned by Clinical Supervisor and coordinates patient care for the assigned caseload.
Communication
  • Completes, maintains, and submits accurate clinical notes regarding the patient’s condition and care. Records pain/symptom management changes/outcomes as appropriate.
  • Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physician orders as required.
  • Coordinates with community health-related persons to implement the care plan; teaches self-care techniques to patient and family/caregiver as appropriate.
  • Provides a safe environment for the patient and supports continuity of care with the interdisciplinary team; attends interdisciplinary group meetings.
Additional Duties
  • Participates in on-call duties as defined by the on-call policy.
  • Ensures arrangements for equipment and other necessary items and services are available.
  • Supervises ancillary personnel and delegates responsibilities when required.
  • Engages in ongoing professional development and continuing education.
  • Fulfills assigned case responsibilities and participates in quality assessment and performance improvement activities.
Job Qualifications
  • Graduate of an accredited school of nursing. Preferred: one to two years of recent acute care experience in an institutional setting, and one to two years of recent home care experience.
  • Current licensure in state and CPR certification.
  • Bachelor’s degree, with one year of hospice care experience preferred.
  • Management experience is not required. Responsible for supervising hospice aides.
  • Excellent observation, verbal and written communication skills, problem-solving skills, and nursing skills per competency checklist.
  • Physical ability to walk/stand for extended periods and to lift, position, and transfer patients and equipment. Adequate visual acuity and hearing.
