We are seeking a compassionate and dedicated Dayshift LPN to join our team at Meadowbrook Care Center, a 128-bed facility located in Northeast Cincinnati. As a Dayshift LPN, you will play a vital role in providing high-quality care to our residents, while working collaboratively with our interdisciplinary team to ensure exceptional outcomes.

• Provide direct patient care, including administering medications, wound care, and treatments as prescribed by physicians
• Conduct vital signs, monitor resident conditions, and report changes to the healthcare team
• Develop and implement individualized care plans, in conjunction with the interdisciplinary team
• Collaborate with rehabilitation staff to develop and implement treatment plans
• Participate in quality improvement initiatives to ensure optimal resident outcomes
• Maintain accurate and timely documentation of resident care and services
• Assist with admissions, transfers, and discharges as needed
• Perform other duties as assigned by the RN or Nurse Manager

• Current LPN licensure in the state of Ohio
• 0 years of experience (new graduates welcome to apply!)
• Strong communication and interpersonal skills
• Ability to work effectively in a fast-paced environment
• Strong attention to detail and ability to prioritize tasks
