Position OverviewShelter House is a community of residents, staff and volunteers working together to provide basic human services for the men and women of the Greater Cincinnati area who are experiencing homelessness. The Rapid Rehousing Case Manager is responsible assisting people in ways the help them quickly exit homelessness and move back into stable community housing.
EssentialJob Duties
- The RRH Case Manager manages a caseload of 35-40 individuals. This individual:
- Houses identified clients within 30 days of program match.
- Connects clients with services including:
Mental health and substance abuse treatment, PCP/Medical Home establishment, employment/employment training. - Identifies and problem solves housing issues with landlords to support housing retention. This includes managing lease signings with clients and landlords, addressing housing issues that arise during program participation and coordinating with housing team for increased engagement/intervention with high-need clients.
- Completes monthly home and office visits with clients, identifying and addressing barriers to prevent future episodes of homelessness.
- Effectively coordinates and communicates with internal/external service providers to support client care.
- Identifies and engages landlords to partner with the program.
- Bachelor's Degree, preferred. Will consider a combination of education and experience.
- Experience in ISP/Case Plan Development and Progress Notes, preferred.
- Knowledge of homelessness, housing-first approach, landlord engagement and/or community resources, preferred.
- Ability to function effectively in a high energy environment.
- Ability to work independently.
- Ability to work flexible hours including weekends and holidays.
Time spent walking- 70%
Time spent sitting- 30%
Must be able to lift 50lbs.
