Minimum Job Requirements
- As of 1/1/24, new practitioners are required to have a Master’s Degree in Dietetics. If eligible or registered prior to 1/1/24, Bachelor’s Degree in Dietetics is required.
- Basic Life Support for Healthcare Providers (BLS)
- CDR Registration & Ohio Licensure
- If needed, dietetic limited permit is accepted upon hire and required to pass the RD exam within 4 months of hire. Applicants not on a limited permit are required to have both licensure and AND registration
- Excellent written, verbal and interpersonal communication skills, computer application skills
- 1-2 years experience in a related field
Positions provide nutritional assessments, counseling and follow up. Oversees diabetes education program. Might provide any of the following based upon program, physician and department needs: develop nutrition awareness programs, provides instruction, treatment, education materials, consultations, diet analysis, cooking demonstrations. In Medical Home Practices (PCMH/CPCI), this role may also work with patients of multiple chronic disease states including Diabetes, Obesity, Heart Disease, Hypertension, and well states to achieve optimal outcomes post-surgery or per consult request per physician.
Job Responsibilities- Performs nutritional calculations (BMI, energy, protein, and fluid needs) and teaches patients how to monitor these values, supporting shared decision‑making within the PCMH/CPCI model.
- Conducts and documents comprehensive nutritional assessments, identifies actual and potential nutrition risks, and provides education and intervention recommendations for all disease states and wellness consults; stays current with payer and managed‑care requirements.
- Evaluates laboratory and diagnostic results for nutritional implications and reviews these results with patients during face‑to‑face visits under the PCMH/CPCI model.
- Demonstrates knowledge of medication–nutrition interactions and collaborates with consulting pharmacists to document goals, next steps, and patient‑reported barriers.
- Actively participates in the Multidisciplinary Team by identifying needs, recommending interventions, collaborating on solutions, traveling to multiple sites for meetings/consults, and completing all EMR documentation and billing.
- Assists with data collection to help the practice measure return on investment, provides quarterly patient‑progress statistics, and follows “incident‑to” billing requirements precisely.
