COMMUNITY HEALTH WORKER FOR AMBULATORY/OUTPATIENT
Job Responsibilities- Work collaboratively with care team of doctors, nurses, and social workers, among other specialties.
- Coordinate care for patients and families, including medical and social needs.
- Complete face to face visits, evaluate care plan progress; communicate results/coordinate next steps with the care manager.
- Monitor ongoing needs of the patients/families to remove barriers, ensure efficiency and help improve access to care and resources.
- Schedule appointments for members/families.
- Complete required documentation linked to the care plan.
- Provide updates to families regarding patient condition and progress; refer families to appropriate resources for information to answer questions and address needs.
- Consult with care team members to address complex issues.
- Supports population outreach to identify population(s) at risk and assist with closing population gaps.

Medical Office Building, 3440 Burnet Ave., Cincinnati OH 45229 - Schedule:
Day Shift/40 hours per week, Monday-Friday 8a-4:30p. - Schedule hours can be flexible if we meet patient needs.
- No evenings, weekends or holidays; no on-call.
- Candidates must have a high school diploma or equivalent
- 2-5 years of work experience in a related job discipline
- Have a reliable vehicle/valid driver's license
- Current Community Health Worker (CHW) certification in the state of Ohio is preferred but not required.
