General Summary
The Director of Social Services is responsible for directing the Social Services Department and performing the duties of the facility licensed social worker.
Responsibilities- Identifies the medically related social services of the resident and ensures that emotional, spiritual and psychosocial needs are met.
- Provides social services to residents requiring such services or makes referrals to Social Agencies if necessary.
- Makes arrangements for financial assistance for residents to appropriate agencies.
- Ensures that the resident, family, and responsible person are fully informed of the resident’s personal and property rights.
- Records pertinent social data about personal and family problems medically related to the patient’s illness and care, and of action taken to meet the resident’s needs.
- Maintains records of each resident referral to outside resources.
- Ensures the confidentiality of all residents’ social and medical information.
- Assists residents and families in the discharge planning process and arranges appropriate community assistance after discharge.
- Conducts an initial social service assessment of all admissions, completes the social service sections on the MDS, and maintains current and up-to-date social service progress notes on all residents.
- Attends all resident interdisciplinary care meetings and coordinates resident and/or responsible party attendance at these meetings.
- A graduate of a bachelor’s program in social work accredited by the Council on Social Work Education. Meets personnel qualification stated in 418.114(b)(3).
- A graduate of an associate’s program in social work accredited by the Council on Social Work Education.
- Minimum of one (1) year experience in health care, hospice experience preferred. Understands hospice philosophy and issues of death/dying.
- Experience in a long term and hospice care preferred.
- Demonstrates good verbal and written communication and organization skills.
- Possesses and maintains current state licensure.
- Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
