Are you a skilled clinical pharmacist looking to make a direct impact on patient care in a hospital setting? We are seeking a dedicated and knowledgeable Clinical Pharmacist to join our multidisciplinary team and play a crucial role in ensuring safe, effective, and individualized medication therapy for our patients.
About the RoleAs a Clinical Pharmacist, you will collaborate closely with physicians, nurses, and other healthcare professionals to optimize medication therapy and improve patient outcomes. You’ll provide expertise in drug therapy management, medication reconciliation, and patient education, ensuring our patients receive the highest standard of care.
In this role, you will:
- Ensure the appropriateness of all elements of pharmaceutical care while adhering to policies and procedures.
- Supervise supportive team members to maintain accuracy and efficiency in distributive services.
- Serve as a drug information resource for Tri Health providers.
- Engage in direct patient interactions, precept pharmacy residents and student pharmacists, and contribute to department projects in collaboration with Pharmacy Leadership.

- Rotates weekly between Tuesday and Wednesday shifts.
- Day shift from 7:30 AM - 6:00 PM
- Required to work every 6th weekend and holiday
Job Requirements
✅ Bachelor's Degree in Pharmacy
✅ Active pharmacist license in the state of Ohio
✅ Strong knowledge of prescription formulas and dosage calculations
✅ Working knowledge of all Ohio Board of Pharmacy regulations
✅ 1-2 years of experience as a Pharmacist (inpatient or infusion pharmacy experience preferred)
- Patient-Centered Care: Review patient information, assess pharmaceutical needs, and recommend appropriate treatments. Provide education/counseling on drug therapies.
- Ensuring Medication Safety: Follow the seven patient "rights"—right patient, right dose, right route, right time, right drug, with the right information and documentation. Report medication errors and support risk management strategies.
- Operational Excellence: Supervise drug distribution processes, oversee inventory management, and ensure timely order fulfillment.
- Continuous Improvement & Leadership: Identify and implement process improvements. Participate in committees, train new hires, and mentor pharmacy students and residents.
Strong interpersonal, analytical, organizational, and problem-solving skills. Ability to work collaboratively with diverse multidisciplinary teams.
