Community Health Worker at Hopple Street Clinic

Job Responsibilities- Work collaboratively with doctors, nurses, social workers, and other specialties.
- Coordinate medical and social care for patients and families.
- Conduct face‑to‑face visits, evaluate care plan progress, and communicate results to the care manager.
- Monitor ongoing needs, remove barriers, and improve access to care and resources.
- Schedule appointments for members and families.
- Complete required documentation linked to the care plan.
- Provide updates to families on patient condition and progress; refer families to appropriate resources.
- Consult with care team members to address complex issues.
- Support population outreach to identify at‑risk populations and close population gaps.
- Shift: Day Shift, 40 hours per week (Monday‑Friday 8 a.m.–4:30 p.m.).
- Schedule hours may be flexible to meet patient needs.
- No evenings, weekends, or holidays; no on‑call.
- Location:
2750 Beekman Street, Cincinnati, OH 45225.
- High school diploma or equivalent.
- 2‑5 years of related work experience.
- Valid driver’s license and reliable vehicle.
- Current Ohio Community Health Worker certification preferred but not required.
