• Work collaboratively with doctors, nurses, social workers, and other specialties.
• Coordinate medical and social care for patients and families.
• Conduct face‑to‑face visits, evaluate care plan progress, and communicate results to the care manager.
• Monitor ongoing needs, remove barriers, and improve access to care and resources.
• Schedule appointments for members and families.
• Complete required documentation linked to the care plan.
• Provide updates to families on patient condition and progress; refer families to appropriate resources.
• Consult with care team members to address complex issues.
• Support population outreach to identify at‑risk populations and close population gaps.

• Shift: Day Shift, 40 hours per week (Monday‑Friday 8 a.m.–4:30 p.m.).
• Schedule hours may be flexible to meet patient needs.
• No evenings, weekends, or holidays; no on‑call.

• High school diploma or equivalent.
• 2‑5 years of related work experience.
• Valid driver’s license and reliable vehicle.

• Current Ohio Community Health Worker certification preferred but not required.
