We are currently recruiting an experienced Physical Therapist for Ohio Oaks Nursing Center, with an option for FT hours at a nearby facility.
SIGN ON BONUS 2,500 for the Full-time schedule and travel reimbursement!

• Plans develop, organize, implement, evaluates, and directs physical therapy, as well as its programs and activities, by current rules, regulations, and guidelines that govern the long-term care facility.
• Develops and maintains physical therapy standards.
• Determines the staffing needs of the physical therapy department necessary to meet the needs of the residents.
• Assists the physician in evaluating the patient's level of function.
• Performs patient tests, measurements, and evaluations - such as range of motion and manual muscle tests, gait and function analysis, and body parts measurements - and records and evaluates findings to aid in establishing or revising specifics of treatment programs.
• Plans and administers prescribed physical therapy treatment programs for patients to restore function, relieve pain, and prevent disability following disease, injury, or loss of body parts.
• Performs and submits required documentation of patient evaluations, plans of care, clinical notes, and progress reports.

• Holds a current license as an Physical Therapist Ohio.
• 1 year experience as a Physical Therapist specializing in Skilled Nursing Facility (SNF) Long Term Care (LTC).
