Full‑time position offering a great opportunity to provide community‑based therapy.

• Perform comprehensive diagnostic assessments.
• Provide patient visits as scheduled.
• Ensure treatment plans are up to date for each client.
• Complete comprehensive and succinct progress notes.
• Communicate and follow up as needed regarding the client’s status.
• Work collaboratively with those providing case management and/or psychiatric services.

• Bachelor’s degree in Social Work or a related field with one year of therapy experience or a Master’s degree in Social Work or Professional Counseling.
• Social Work/Professional Counseling License or equivalent: LSW, LISW, LISW‑S, LPC, LPCC, LPCC‑S, LMFT, LIMFT, LIMFT‑S.
• Experience in therapy and counseling is preferred.
• Experience with in a home setting is preferred.
• Valid driver’s licence and reliable transportation.
• Willingness to travel throughout assigned service area.

• Comprehensive training.
