Job Summary

We are looking for a Home Care Aide/ Caregiver/ DSP to join our team! You will be directly working with patients, following a one-on-one care plan in patient homes. You operate with a big heart, not only providing personalized and attentive care but by building genuine relationships with those we serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs.

Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health Care experience is a plus.

Responsibilities

Support people to achieve personal outcomes that characterize a high quality of life

Understand. Follow through, and document support plans, goals, activities and progress.

Support clients with daily living skills and personal support needs.

Facilitate activities that provide clients with social capital and connections to the community.

Maintain confidentiality and support privacy at all times.

Administer medications accurately and reliably.

Monitor medical conditions thoroughly and consistently

Ensure that clients are free from abuse, neglect and exploitation.

Complete necessary reporting and documentation on time.

Support clients with daily living skills and personal support needs

Follow outlined care plans and financial supports

Report expenses on ledger and assist caregivers with client financial allowances.

Be a safe and conscientious driver when operating company vehicles or transporting clients.

Performs other related duties as assigned.

Qualifications

High school or equivalent (Required)

Ability to pass criminal/registry background checks

At least one year prior professional experience

Driver's License (Required)

Ability to pass and maintain required trainings such as CPR/First Aid, Medication Administration and Positive Behavioral Support Training

Must have exceptional communication skills including written, verbal, etc.

Must possess superior judgment, listening, and decision-making skills

Ability to work with a variety of personalities.
