GCBHS has immediate openings for Senior Case Managers to help adults who are experiencing severe and persistent mental illness and who live in the community. Join our mission to help individuals with mental illness and addictions lead healthy and productive lives. Our teams are located in Cincinnati and Amelia, Ohio, serving residents in Clermont and Hamilton County. This position is a great starting point for a Case Manager with 2‑3 years of experience seeking career advancement and a stepping stone to a supervisory role.

- Practicum supervision, license supervision, and continuing education credits.
- Provide case management, Community Psychiatric Support Treatment (CPST) services, and Therapeutic Behavioral Services (TBS) to individuals with severe mental illness in an outpatient setting and in the community; may also treat individuals with substance use disorder as a secondary diagnosis.
- Initiate, coordinate, and monitor evaluations necessary to identify each client’s needs in areas including living arrangements, financial issues, benefit status, legal status, physical/psychiatric health, daily living activities, vocational/educational goals, social supports, and leisure/recreation.
- Use assertive outreach to engage clients in identifying and expressing their own needs, offer services to meet those needs, and re‑assess outcomes regularly.
- Acquire knowledge of mental health treatment and basic needs resources, informal and formal agency linkages, client service use, associated costs, and alternative resource opportunities.
- Serve as a liaison with other service staff.
- Delegate services to secondary and supportive service providers, and monitor and evaluate the outcomes of those services.
- Assist clients in achieving goals and maximizing independence and productivity through support, teaching, and training in personal and community resources.
- Review and monitor services and activities, documenting progress according to organizational processes.
- Assist in onboarding and initial training of new care managers.
- Act as a resource for less experienced care management staff.
- Possibly represent GCBHS in partnerships and shadowing experiences, such as CIT police shadowing.
- Provide services to individuals in Cincinnati/Hamilton County or Amelia/Clermont County.
- Bachelor’s or Master’s degree in a Human Services field or high school diploma plus a minimum of three years of experience in Behavioral Health, Mental Health, or a related Human Services field.
- Two years of experience as a Mental Health Care Manager.
- Experience in social services and behavioral health.
- Frequent driving of the employee’s personal vehicle; a valid driver’s license, vehicle, and insurable driving record are required.
