ScheduleMonday through Friday: 8:30 a.m. – 5 p.m. Must be available to work weekends and evening hours as needed to support, guide and assist staff and to meet the various needs of the program.

ProgramThe Enhanced Care Management (ECM) program provides person‑centered care coordination to individuals with complex medical, behavioral health, and social needs. Services are delivered by a multidisciplinary team and include care coordination, referrals, advocacy, and linkage to medical, behavioral health, and community resources. The program emphasizes whole‑person, culturally responsive, and trauma‑informed care to improve health outcomes and member stability.
RESPONSIBILITIESUnder the supervision of the Program and Department Directors, the Program Coordinator is responsible for the overall operations, coordination, and day‑to‑day oversight of the ECM program serving SBCS clients and the community. The Program Coordinator provides guidance and support to ECM staff in the development and achievement of team goals and objectives and collaborates with the Program Director to ensure adherence to the contract scope of work.
Services are delivered within a team‑based framework and aligned with the agency’s mission, goals, and objectives.
