Overview
Homebase Medical is focused on optimizing the quality of life for homebound older adults, enabling them to live independently and with dignity provide house calls and wrap-around services for clients with complex health needs and collaborate with SCAN, a not-for-profit organization that has supported older adults for over 45 years. The position is in the Central San Diego / Chula Vista area and is for a Nurse Practitioner or Physician Assistant to engage in Medicare Wellness Visits at contracted partner clinics and occasional home visits.
Responsibilities- Perform Wellness Visits in partner primary care clinics, facilities, and in-home settings within the Chula Vista community.
- Travel to designated facility locations in your assigned geographic area.
- Review patient medical and family history, current providers, and prescriptions.
- Take and record height, weight, blood pressure, and other routine measurements.
- Provide personalized health advice and health education to patients, focusing on prevention, health promotion, and management of chronic conditions.
- Review risk factors and potential treatment options; communicate complex medical information with clarity and compassion.
- Provide referrals to appropriate medical resources and utilize our EMR system to document visits accurately and timely.
- Demonstrate knowledge of HCC coding and perform other duties as assigned.
- New graduates will be considered with at least 5 years of RN experience in primary care, hospice, or urgent care/outpatient settings.
- Graduate of an accredited Physician Assistant or Nurse Practitioner program.
- Current, unrestricted NP or PA license to practice in California, including DEA license.
- Minimum of 1 year of previous experience using an EMR system.
- Spanish speaking preferred.
- Strong working knowledge of HCC coding and computer skills (MS Office and Outlook).
- Excellent verbal and written communication skills.
- Current driver’s license, auto insurance, and reliable transportation.

- Base Salary: $140,000 – $150,000 annually plus bonus
