Medi Home Health & Hospice a division of Medical Services of America, Inc., currently seeks an experienced Full-Time Medical Social Worker for our Hospice patients in Christiansburg, Blacksburg, Radford, and Pulaski (Montgomery), VA.

• Provides social work services in accordance with the physician’s plan of treatment.
• Works with the interdisciplinary team directly, or through coordination of team members, to provide appropriate assessment and support patients and families.
• Provides assessment for normal and complicated grief and provides bereavement counseling as appropriate.
• Serves as a liaison with other community resources.

Job Requirements

• Master's or Bachelor's level social work degree from a school of social work accredited or approved by the Council of Social Work Education.
• Currently licensed to practice as a social worker in the state of VA.
• Minimum of one-year Hospice experience preferred.
• Valid Driver's License.
