Overview
Registered Nurse, Primary Care - (Multi-site) – EAST Region. Fraser Health Authority is seeking qualified candidates to join a casual (on-call) team across Urgent Primary Care Centres in multiple locations.
Salary: CAD $41.42 - $59.52 per hour.
Responsibilities- Provides direct nursing care to clients receiving scheduled or urgent primary care; assesses clients and collects information using observation, communication, data review and physical assessment; determines nursing interventions and collaborates with other clinical staff as needed.
- Processes physician or nurse practitioner orders by initiating unit standing orders, administering medications, carrying out treatments and monitoring effects.
- Establishes a therapeutic relationship with clients to support autonomy and informed decision-making, while protecting dignity and privacy.
- Screens referrals, conducts client assessments, plans inter-professional care, and coordinates with primary care providers, family/supports, and other health care professionals to ensure timely access to services.
- Develops comprehensive care plans with the inter-professional team and supports transitions to primary/community care providers and community agencies.
- Delivers direct and planned/unplanned client care following applicable guidelines and evidence-based best practices; explains care to clients and families.
- Reviews and updates care plans with the team to determine timing and referrals to improve outcomes; supports transitions across the care continuum.
- Discusses findings with clients/families; plans actions, sets goals, and promotes self-management and health-related quality of life.
- Supports outreach nursing care to clients in home or other settings; provides education and information to clients and families about care plans and community resources.
- Maintains clinical records and statistical information in accordance with policies and procedures.
- Participates in quality improvement, risk management, and professional development activities; supports orientation and education of nursing staff and students as appropriate.
- Assists in the development and revision of standards of care, policies and procedures; contributes to quality improvement initiatives.
- Performs other related duties as assigned.
Education and Experience
Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
One (1) year recent related clinical experience in acute care, urgent/emergency care or related area including care of children, youth and adults in acute or community/outpatient settings, or an equivalent combination of education, training and experience.
Valid BC Driver’s license and access to a personal vehicle for business-related purposes, as required.
Skills and Abilities- Demonstrated ability to complete initial and ongoing client assessments and provide nursing care through therapeutic interventions.
- Effective verbal and written communication; ability to document and convey information clearly.
- Ability to synthesize data from multiple sources to problem-solve; knowledge of broad health care services and community resources.
- Promotes client-focused care with sensitivity to diverse cultures and preferences.
- Ability to prioritize and manage multiple clients with varied health issues; capacity to teach clients and others about health care and self-management.
- Ability to work independently and as part of an interprofessional team; operate related software and equipment.
- Physical ability to perform duties as required.
