Job Description
Contingent – Jackson, Pike, Scioto, and Gallia counties.

We are seeking a Palliative Care Nurse who will share in our vision to advance better living and care for seniors!
Essential Functions- Conducts physical exams and comprehensive medical and social history of individuals, including those who are healthy and those with acute illnesses and chronic disease; makes independent decisions regarding management and treatment.
- Orders, performs, and interprets laboratory tests for screening and diagnosing.
- Prescribes and orders appropriate diet, medications, and other treatment such as physical therapy.
- Performs therapeutic or corrective measures, including urgent care.
- Refers individuals to appropriate specialist nurses, physicians, or other health care providers.
- Provides information, instruction, and counseling on health maintenance, health promotion, social problems, illness prevention, illness management, and medication use.
- Evaluates effectiveness of instruction and counseling, providing additional instruction as necessary; initiates and maintains follow-up for noncompliant patients.
- Serves as preceptor for medical, nursing, NP, or PA students.
- Consults with other health care providers about established clients admitted to hospital, home care, rehabilitation, or nursing homes.
- Corresponds with insurers, employers, government agencies, and other providers as necessary.
- Manages care of clients, develops treatment plans and follow-up, monitors progress, and determines necessary referrals, hospital admission, ER visits, or resolution of illness.
- Assesses social/economic factors for each client, tailoring care to those factors.
- Maintains familiarity with community resources and connects clients with appropriate resources.
- Contracts with clients regarding provider and client responsibilities.
- Maintains an acceptable level of productivity to meet facility goals.
- Medical &rx (both PPO & HDHP w/HSA) – including coverage for fertility.
