Practice OverviewJoin a mission-driven oncology team dedicated to delivering high-quality, community-based cancer care close to home. The oncology department includes:
- 6 Medical Oncologists
- 2 Radiation Oncologists
- 3 Certified Nurse Practitioners
- 1 additional 0.8 FTE CNP (this position)
This role supports continued growth in oncology patient access, survivorship care, and hematology disease management while maintaining provider workload balance.
Position SummaryThis is a primarily outpatient oncology role focused on follow-up care, hematology management, survivorship, chemotherapy education, and supportive oncology services.
You will function in a highly collaborative model with increasing autonomy while maintaining close physician partnership.
Key Responsibilities- Conduct independent and collaborative outpatient oncology visits
- Manage long-term oncology follow-up and survivorship care
- Provide hematology disease management
- Deliver chemotherapy education and treatment monitoring
- Coordinate symptom management and supportive care
- Participate in multidisciplinary oncology team planning
- Support patient education and care navigation
- Assist with inpatient oncology consults during assigned coverage weeks
- Maintain high-quality documentation and evidence-based care standards
- Inpatient Coverage: 1 week approximately every 5–6 weeks
- Daytime First Call:
During assigned inpatient week - Evening/Weekend Call:
Covered by Medical Oncologists
- 8–12 patients per day
- Mix of follow-ups, surveillance visits, hematology management, and treatment support
Each CNP is assigned a dedicated caregiver daily for rooming, messaging, and call handling.
Practice Model- Highly collaborative physician–APP partnership

- Oncology or hematology experience strongly preferred
- Prior APP experience preferred due to specialty complexity
- New graduates considered with strong oncology interest and clinical foundation
