Registered Nurse- Patient Aligned Care Team

This position as a Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) will float to any Primary care location as needed to provide necessary coverage and is responsible and accountable for individual nursing practice and determines the appropriate delegation of tasks consistent with the nurse's obligation to provide optimal care. Incumbent will provide direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department.

Duties and responsibilities include but are not limited to:

* Influences care outcomes by collaborating with members of the interdisciplinary team.

* Administers medications and procedures per established policies and guidelines.

* Applies clinical knowledge/judgment to lead staff involvement in planning, decision-making, and evaluating outcomes within the immediate practice setting.

* Improves Veteran/patient driven care by resolving experience issues such as coordination of care, timeliness or process issues, common themes of dissatisfaction, or environment of care concerns for the immediate practice setting.

* Demonstrates knowledge over a wide range of job responsibilities, effectively communicating with individuals and groups to build a strong sense of teamwork and purpose.

Pay:
Competitive salary, regular salary increases, potential for performance awards
