We are seeking a dedicated Licensed Practical Nurse (LPN) to join our supportive and professional nursing team.

The LPN at Hopewell Grove provides direct nursing care to residents under the supervision of an RN or physician. This role is essential in ensuring residents receive safe, compassionate, and timely care while maintaining compliance with all regulatory standards.

• Provide direct nursing care, including medication administration and treatments
• Monitor resident conditions and report changes to the RN or physician
• Accurately document care, medications, and observations
• Assist with care planning and resident assessments
• Collaborate with CNAs and interdisciplinary team members
• Ensure compliance with state regulations and facility policies
• Promote a safe, clean, and respectful environment for residents

• Current LPN license in good standing
• Previous experience in long-term care or skilled nursing preferred
• Strong clinical and documentation skills
• Compassionate, patient-focused approach to care
• Ability to work effectively as part of a team
