Position Summary
Position Summary: The Clinical Pharmacist Specialist participates in the provision of rational, cost-effective pharmaceutical care utilizing sound pharmaceutical and professional ethics. Decisions are made independently or in collaboration with others. This position has access to patient, financial and organizational confidential information, non-controlled and controlled medications and certain hospital funds. The Clinical Pharmacist Specialist works with patients in all age groups and performs non clinical duties if needed.
Also responsible for assisting Adena Health System leaders in the following areas: quality process improvement, education of students, and collaboration with other departments, professionals, vendors, and customers to achieve operational goals. A strong self-drive along with a consistent display of high ethical standards are essential. The Clinical Pharmacist Specialist may be assigned to any of the various areas of Pharmacy such as but not limited to Ambulatory or Acute.
A commitment to continuous learning, highly effective communication skills, and an advanced knowledge of medication therapy management are highly critical.
Minimum Qualifications Required Educational Degree:
Graduate of a BS or Pharm
D ACPE Accredited Program in Pharmacy Major/Area of Concentration:
Pharmacy
Preferred Education:
Required Certifications, Credentials and Licenses: current license to practice Pharmacy in the State of Ohio. Applicable current Pharmacy Board Certification and/or PGY1 residency. Preferred Certifications, Credentials and Licenses: BLS ACLS and Immunization Certification dependent on area assigned
Required Experience:
3-5 years Applicable experience in pharmacy with demonstrated history of clinical leadership and initiative. Preferred Experience
Job Specific Knowledge, Skills & Abilities Must possess effective verbal and written communication skills and demonstrate organizational abilities. Previous computer experience in word processing, spreadsheets, and Pharmacy IT systems desired.
Job Specific Essential FunctionsEnsures appropriate, cost effective drug therapy and monitoring for assigned patients through continuous learning and maintenance of applicable competencies and certifications. Ability to learn and utilize various point of care testing devices and direct patient care procedures as deemed necessary.
Appropriately completes and documents clinical interventions and outcomes.
Assists Pharmacy Manager in implementing, and monitoring clinical protocols and guidelines to enhance the delivery of evidence based pharmacotherapy.
Helps develop, maintain and ensure clinical pharmacy initiatives through multidisciplinary teamwork and personal execution.
Exhibits excellent interpersonal and customer service skills in all interactions with internal and external customers.
Provides education on drug therapy, disease state management, lifestyle modifications, adverse drug reactions and medication compliance to internal and external customers along with appropriate monitoring.
Works to improve medication therapy quality through integration/expansion within various health system departments.
Attends of leads Health System committees and provides drug education inservice/presentations as needed. Serves as a Pharmacy Student and Resident Preceptor while maintaining proficiency with educational/mentorship skills.
